Position Type: Full-Time, Remote-Staff residing within the state of Ohio will be expected to attend meetings and events as required by their position or requested by their supervisor.  These meetings may be held in Montgomery, Miami, and Greene Counties.

Role Overview: We are seeking a highly organized and detail-oriented individual to assume the role of the Case Coordinator for the Bridges program. This position, which reports to the Senior Regional Coordinator, is ideally suited for an individual who possesses a strong passion for advancing the well-being of transition-age youth and excels at cultivating robust relationships with community stakeholders and key professionals.

The Case Coordinator will be accountable for the Warm Handoff of youth leaving foster care and entering our Bridges program. The Case Coordinator’s responsibilities will include close collaboration with Public Children Services Agency (PCSA) caseworkers, community stakeholders, and the Bridges team to ensure seamless transitions. Furthermore, you will provide essential support to young adults during this critical time of aging out of foster care. Your initiatives will foster partnerships and establish meaningful relationships that enhance the transition outcomes for youth.

Key Responsibilities

  • Collaborative Support: Establish regular meeting cadence with the Regional Coordinator(s) and Intake and Referral Coordinator to build rapport, establish processes, and maintain consistent communication about transition-age youth.

 

  • Stakeholder Support: Establish communication with the PCSA and Independent Living (IL) teams to develop a rapport, relationship, and provide education about their role during the Warm Handoff (WHO) process.  Partner with the PCSAs and Independent Living teams to develop processes, outline a communication plan, and institute a process to promote collaborative work in successfully supporting and transitioning youth from foster care to Bridges. Serves as the primary contact prior to and during the initial stages of the WHO process. Inform the PCSA and IL teams about the process and its expectations.

 

  • Young Adult Support: Serve as the primary contact prior to and during the initial stages of the WHO process.  Informing young adults about the process and its expectations.  Gather information from the young adults about their needs, strengths, goals, eligibility for the program, and barriers they are encountering.  Identifying applicable resources to address the young adults’ immediate needs, securing housing, and ensuring they are actively working towards meeting Bridges’ eligibility requirements.

 

  • Community-based Support:  Attend all meetings involving the young adult, which include but are not limited to court hearings, transition meetings, OhioRISE Child and Family team meetings, meetings involving county Boards of Developmental Disabilities, job development, residential treatment, guardianship, and/or probation.  Collaborate with various community stakeholders, including foster caregivers, CASA/GAL, probation officers, residential service providers, job developers, clinical therapists, mental health case managers, and housing resources in Montgomery, Miami, and Greene Counties.

 

  • Key Performance Indicators (KPIs):  Develop and implement Key Performance Indicators or KPIs for the WHO process which includes data collection and analysis of touchpoints with the Montgomery, Miami, Greene County youth emancipating from care, PCSAs, and stakeholders to evaluate and understand their impact on unique referrals and enrollments.  This data will be shared with the Regional Coordinators to help guide their communication with their agencies about capacity adjustments to accommodate new referrals.  Design, conduct, and evaluate surveys to assess the experiences of young adults, PCSAs, and IL teams throughout the WHO process.

 

Qualifications:

  • A minimum of an Associate Degree in human services, criminal justice, or related field and experience working in a social service or community-based organization      
  • Familiarity with local community resources and networks
  • A commitment to advancing equity  
  • Professional attitude and appearance   
  • Excellent written and verbal communication skills   
  • Highly organized; resourceful and proactive; pays close attention to detail   
  • Strong time-management skills, with the ability to prioritize tasks   
  • Ability to handle sensitive and confidential information
  • Crisis intervention training or experience preferred
  • Ability to collaborate with multiple teams of people simultaneously  
  • Ability to work independently and as a member of a team
  • Strong problem-solving and critical-thinking skills
  • Ability to collect, analyze, and interpret data
  • Ability to quickly gain proficiency using other software and web-based platforms as needed.
  • Proficiency with Microsoft Office 365 products (Outlook, SharePoint, Teams, Word, PowerPoint and Excel)
