At First Mile Care we’re building the nation’s first affordable, scalable, and sustainable platform to improve the lives of people who have prediabetes. Our goal is to half the number of people in the U.S. with prediabetes by 2028. How? By scaling the CDC’s proven Diabetes Prevention Program (DPP). We are committed to creating a nationwide network of First Mile Coaches in the vast majority of the U.S.’s 42,000 ZIP codes: local heroes trained to help their fellow community members make the right decisions on a daily basis so they can gain control of their health and reverse prediabetes.

THE POSITION

Diabetes Prevention Program lifestyle coaches lead groups of participants through a year-long program to reduce the risk of diabetes in individuals with prediabetes or at high risk for developing type 2 diabetes. Using a CDC approved, evidence-based curriculum, coaches provide support and guidance to participants and help them develop and maintain the skills needed to adopt healthy eating and physical activity habits.

Coaches are responsible for:

  • Facilitating weekly or bi-weekly sessions with a group of 8-15 participants through the year-long Diabetes Prevention Program. Due to COVID-19 advisories, groups are temporarily meeting virtually via video conference. Once advisable, groups will meet in person. 
  • Providing curriculum to class participants.
  • Maintaining accurate attendance logs.
  • Assisting with participant recruitment activities.

REQUIRED QUALIFICATIONS

  • Have successfully completed Diabetes Prevention Program Training or successful completion of the Diabetes Prevention Program Training upon hire required.
  • Commit to a year-long coaching position.
  • Familiar and proficient with motivational interviewing and group facilitation.
  • Be passionate about helping community members live a healthier lifestyle.
  • Be committed to and supportive of the First Mile Care mission, with a passion for helping people in your community.
  • Have strong communication and relationship building skills, the ability to build community within a group, and have empathy for health seekers.
  • Ability to interact positively with people of all ages and abilities.

PREFERRED QUALIFICATIONS

  • 2+ years of experience in health education and/or coaching with individuals and groups for disease prevention, health promotion and/or behavior change.
  • Knowledge of basic health, nutrition and fitness guidelines.

Part-time, Contract / Pay from $22.00 per hour
