Job Summary:
The Renal Dietitian is responsible for providing ongoing care for all nutrition aspects of ESRD patients.

Qualifications:

  • Registered Dietitian with the Commission of Dietetic Registration

  • Minimum one year clinical experience as a Registered Dietitian

  • Licensed in states where licensure is available. 


    Job Specific Accountabilities:

1. Orient new patient and family member or significant other as appropriate to nutritional care for the dialysis patient.

  1. Performs an initial nutritional component to the Interdisciplinary Assessment and Care Plan for each new ESRD patient within 30 days of admission or 90 days for transfers. Instructs the patient and family member or significant other in diet restrictions as prescribed by the physician. Ensures that the ESRD patient understands the prescribed diet.

  2. Encourages the patient to comply with the diet as prescribed while teaching the patient the significance of outcomes relative to on-going health and stability.

  3. Cooperates with attending physicians in ongoing evaluation of patients' nutritional needs and recommends necessary changes in therapeutic diets.

  4. Works with other team members in making necessary arrangements for nutritional support (e.g. meals on wheels, nutritional supplements).

  5. Educate patients and their families on an on-going basis regarding elements of prescribed diets based on lab outcome data and other physiological parameters.

  6. Monitors and documents adherence and response to diets; emphasis is placed on outcome measures relevant to nutritional status.


JOB DESCRIPTION


  1. Recordkeeping

    Responsible for making entries in the patient's medical record in accordance with the requirements as established in the Medicare Conditions of Coverage as a minimum.

  2. Communication with Others

    1. Attends patient care conferences and staff meetings on a regular basis.

    2. Participates as a member of the facility's Continuous Quality Improvement committee.

    3. Participates, when necessary, in individual patient-family conferences, along with the Medical Director or attending physician and social worker.

    4. Conducts in-service training for dialysis staff and patient education programs as necessary.
