TITLE: Case Manger  

Department: Social services 

Reporting to:  Program Director 

FLSA Status: Non- Exempt 

Salary:  55k  annually  

Schedule: 9am-5pm 

Location:  163rd Street Council, locations 

 

 

 

Mission Statement  

The 163rd Street Improvement Council empowers individuals and families including veterans, the formerly incarcerated, LGBTQIA+ individuals, and those with physical, cognitive, or developmental challenges by providing housing and connecting them to the resources, support, and opportunities needed to transition from homelessness to stable, healthy, and fulfilling lives.  

  

History  

Founded in 1966, the 163rd Street Improvement Council, Inc., was established to address the chronic, systemic lack of resources in the South Bronx. As pioneers in housing justice in the Bronx, we began by developing affordable housing for some of our most vulnerable Bronx residents.     

 

Today, we are a tenant-centered organization driven by our core values of Strength, Support, and Stability. We offer a continuum of care from transitional housing to congregate living designed to promote dignity, foster independence, and support long-term stability for those we 

 serve. 

 

The Role: 

 
The Case Manager provides direct support to clients by assessing their needs, developing service plans, connecting them with community resources, and monitoring progress toward self-sufficiency. Working in a non-profit setting, the Case Manager serves as an advocate, counselor, and liaison to ensure clients receive the services and support necessary to overcome barriers and achieve their personal, social, and economic goals. 

Key Responsibilities: 

  • Conduct intake interviews and needs assessments with clients to identify strengths, challenges, and goals. 

  • Develop and implement individualized service or treatment plans. 

  • Provide ongoing case management, including referrals to housing, healthcare, employment, education, financial assistance, and social services. 

  • Advocate for clients within community systems and assist in navigating complex services. 

  • Maintain regular contact with clients to monitor progress and provide support. 

  • Collaborate with community partners, government agencies, and internal staff to coordinate holistic care. 

  • Maintain accurate and timely case notes, records, and data in compliance with agency and funder requirements. 

  • Facilitate support groups, workshops, or life-skills training as needed. 

  • Respond to crises and provide short-term intervention and problem-solving support. 

  • Track client outcomes and prepare reports for program evaluation and funding compliance. 

Qualifications: 

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (Master’s preferred in some programs). 

  • Experience in case management, social services, or community-based work (non-profit experience preferred). 

  • Strong knowledge of local resources and service systems (housing, healthcare, benefits, employment, etc.). 

  • Excellent communication, advocacy, and problem-solving skills. 

  • Ability to manage multiple cases, meet deadlines, and maintain confidentiality. 

  • Cultural competence and sensitivity when working with diverse populations. 

  • Proficiency with Microsoft Office and case management databases.

Knowledge:    

Job duties require a general familiarity with 163rd Street Improvement Council Inc programs, services, and operations.     

   

Compensation:   

Competitive salary, incentive and benefits will be offered, commensurate with the role and the final candidates experience.

Working Environment:   

This position will be performed in a professional office. This position routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This position is subject to travel.
