LO3 Energy Employee Benefits Summary

• Health – multiple plans available (varies by location) with company paying 75% of the premium of selected plan
• Dental – multiple plans available with company paying 75% of the premium of selected plan up to the following monthly limits
• Vision – multiple plans available; optional program with premium paid by employee
• Disability – company pays 100% of insurance premium. Insurance pays 50% of employee’s salary when on disability.
• Life – company pays 100% of insurance premium for $20,000 life insurance plan; employees may purchase higher limits at an additional cost paid by the employee
• PTO (Paid Time Off) – unlimited PTO; subject to manager approval
• Holidays – 10 paid holidays per year
• 401K plan - available for employee contributions
• Stock Options – stock options are granted to enable employees to participate in the company’s success by holding equity in the company
