The Primary Care Advanced Practice Provider is an essential healthcare provider responsible for delivering comprehensive primary care services. This role involves diagnosing and managing acute and chronic conditions, prescribing medications, performing preventative screenings, and coordinating patient care. The Primary Care Provider will play a key role in improving patient outcomes by providing high-quality, patient-centered care.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Obtain and accurately document a thorough medical history in the electronic medical record.

  • Diagnose and manage acute and chronic health conditions, utilizing evidence-based practices.

  • Perform physical exams, order and interpret diagnostic tests, and develop comprehensive treatment plans.

  • Prescribe medications and treatments, ensuring patients understand their usage, side effects, and interactions.

  • Provide preventative medical screenings, advising on recommended screenings and immunizations.

  • Provide education on nutrition, physical activity, and other lifestyle habits to promote overall health and well-being.

  • Coordinate patient care with other healthcare providers, specialists, and services to ensure comprehensive and continuous care.

  • Schedule patients' appointments, tests, and procedures as needed.

  • Consult with supervising physicians or other healthcare professionals for complex patient cases.

  • Maintain accurate and up-to-date patient records, documenting patient interactions, care plans, and outcomes.

  • Ensure compliance with healthcare regulations, standards, and protocols.

  • Carry out additional duties as assigned.

REQUIREMENTS:

  • Current active and unencumbered license to practice as a Nurse Practitioner or Physician Assistant in Texas with willingness to obtain additional licensure as needed

  • Certification as a Physician Assistant (PA-C), Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) if applicable.

  • Must be detail-oriented and able to multitask effectively.

  • Excellent communication and written skills.

  • Ability to work independently and as part of a collaborative environment.

  • Comfortable with learning and using new technology and software.

  • 2+ years of hands-on experience in a primary care setting

PREFERRED EXPERIENCE/SKILLS:

  • Experience in providing virtual care or practicing telehealth (strongly preferred)

  • Experience working in a start-up

  • Fluent in Spanish

EXPECTED HOURS OF WORK:

Monday-Friday, 8am -5pm

This position will require 1 evening per week and 1 Saturday per month of weekend work (total working hours not to exceed 40hr/wk).
