AC Wellness Network is an independent medical practice exclusively dedicated to delivering compassionate, effective healthcare to the Apple employee population at the Apple Wellness Centers in Santa Clara Valley, including the new Apple Park Wellness Center. QUALIFICATIONS
  • California licensed therapist (LCSW, LMFT, PhD, PsyD) in good standing with the BBS or CBP
  • Minimum of 2 years experience providing counseling, cognitive behavioral therapy and other psychotherapeutic interventions; experience in primary care strongly preferred
  • Thorough knowledge of health behavior and adult learning theories and their application
  • Working knowledge of complementary/alternative mental health treatments desirable
  • Current BLS Certification from the American Heart Association

JOB DESCRIPTION
As an integral member of the primary care team, the Behavioral Health Partner works in direct partnership with the primary care physician to help patients improve health and wellness through sustainable behavior change. The Behavioral Health Partner works collaboratively across multiple disciplines to help patients improve mental, physical, and spiritual health. In addition to providing evidence-based assessment, diagnosis and treatment of DSM 5 disorders under the supervision of the primary care physician, the Behavioral Health Partner will identify and address coexisting health concerns including chronic disease management, weight loss, sleep, physical activity, and nutrition.

  • Serve as primary point-of-contact for patients
  • Provide one-on-one patient therapy, coaching, education, and support through various modes of communication
  • Translate scientific language and concepts into understandable information for patients
  • Consult with primary care physician and care team to promote and integrate high quality, patient-centered care
  • Develop, tailor, and disseminate culturally and linguistically competent health education material
  • Link patients with expert consultants when appropriate
  • Collect and analyze data for program evaluation
