The Lead Nurse organizes and oversees activities of clinical team members to ensure compassionate, effective and efficient delivery of high quality clinical care for patients. The Lead Nurse  monitors daily and weekly care coordination needs and implement actions to address those needs. The Lead Nurse performs all steps of the nursing process, with particular focus on high risk patients, including assessing patients, interpreting data, planning, implementing, and evaluating care; coordinating care with other providers; and teaching the patient and family the knowledge and skills needed to manage their care to prevent complications. 


Using the latest in evidence based practice, cutting edge technology and the power of a dedicated, supportive team, the Lead Nurse collaborates with patients and providers to create healthcare that empowers patients to take an active role in their health. Our nurses research, learn, teach and inspire.


Shift: 4x10 | Tuesday-Friday from 8am-6:30pm.


ESSENTIAL JOB FUNCTIONS 


  • Manages population health for care team.
  • Assesses patient health care status at the time of service and through continuum of care, identifying and addressing patient needs among a panel.
  • Collaborates with the team members to manage the plan of care to ensure it is effective efficient and personalized for each patient within their panel. 
  • Provides mentoring and support to the Medical Assistants.
  • Assess, plan, implement and coordinate care for high risk patients; ensure effective link to specialists, as needed.
  • Participates in office visits for high-risk patients.
  • Meets weekly with providers to address care coordination needs.
  • Coordinate care for RN visits.
  • Continually evaluates patient flows and provides intervention to facilitate patient flow to an optimum time line.
  • Anticipates potential problems/obstacles in daily operations and intervenes effectively. Provides input and suggestions for ongoing improvement of operations.
  • Provides input and suggestions for improving operations and quality of care; Program development and design.
  • Implements steps and strategies to improve quality of care in a cost-effective manner
  • Occasionally provides direct RN care to patients within any of the program areas when needed to support health center operations. 
  • Applies the nursing process to evaluate patient health. This may include scheduled visits, walk-in patients and patient telephone calls to the practice.
  • Implements appropriate patient education programs to meet the individual health care needs. 
  • Documents patient interactions following clinic guidelines
  • Continually works to achieve the highest level of patient satisfaction.
  • Maintains the strictest confidentiality.

QUALIFICATIONS AND REQUIREMENTS

  • Current California RN license.
  • Nursing degree from accredited school. Bachelor of Science in Nursing (BSN) Preferred.
  • Minimum 3 years of clinical experience that includes medical management activities.
  • Current Basic Life Support (BLS) for health care provider. 
  • Demonstrates the knowledge and skills necessary to perform all job tasks and per approved  competencies.
  • Demonstrates comfort and enthusiasm for new methods of care delivery using technology
  • Consistently models excellent communication and problem solving skills with all staff, patients, providers and others by using a positive and courteous team approach.
  • Serves as a role model for professional behavior in the delivery of patient care.
  • Demonstrates effective team leadership skills.  Actively promotes a positive team environment.
  • Adheres to HIPAA and privacy-related policies to maintain the privacy and security of patient information.
  • Demonstrate critical thinking and ability to work with minimal supervision.
