The Nurse Coordinator works to create a frictionless experience for all patients patient experience. The nurse performs all steps of the nursing process including assessing patients, interpreting data, planning, implementing, and evaluating care; coordinating care with other providers; and teaching the patient and family the knowledge and skills needed to manage their care to prevent complications. Using the latest in evidence based practice, cutting edge technology and the power of a dedicated, supportive team, the nurse care coordinator collaborates with patients and providers to create healthcare that empowers patients to take an active role in their health. Current Shift Available:
30hrs: 3 x 10 on M/T/W from 7:30am-6:00pm

ESSENTIAL FUNCTIONS & SKILLS
  • Applies the nursing process to evaluate patient health. This may include scheduled visits, walk-in patients and patient telephone calls to the practiceAssess patient health care status at the time of service
  • Collaborate with the team members to implement a plan of care appropriate to the current situation. Obtain medical records as necessary
  • Implement appropriate patient education programs to meet the individual health care needs
  • Document patient interactions following clinic guidelines
  • Continually works to achieve the highest level of patient satisfaction
  • Continually evaluates patient flows and provides intervention to facilitate patient flow to an optimum time line
  • Serves as a role model for professional behavior in the delivery of patient care
  • Demonstrates excellent communication and problem solving skills with all staff, patients, providers and others by using a positive team centered approach
  • Anticipates potential problems/obstacles in daily operations and intervenes effectively
  • Makes suggestion for improving operations and implementing efforts to improve the quality of care in a cost-effective manner
  • Demonstrate comfort and enthusiasm for new methods of care delivery using technology
  • Maintains the strictest confidentiality
  • Other duties as assigned


EDUCATION AND REQUIREMENTS

  • California registered nurse license (RN)
  • Registered Nurse from accredited school
  • 3 years of clinical experience
  • Current Basic Life Support (BLS) for health care provider
